This is one of the main items for Vladimir, as his passive causes AP to give free HP, and HP to give free AP. That means if you get this item, you get free stats from both mods. The Rylai scepter is another such item any Vlad should get for this reason.

Sell it when you need to clear a space to buy the last item, since it's better then any doran's. I think the more important question would be when it's too alte to buy it (when getting zoned etc) and just buy a voidstaff instead.
Note that there probably is no exact time, it all depends on how much MR the enemy team is building.
